Welcome to Your Tax Service Forms & Getting Started Guide
We know that filling out paperwork isn’t anyone’s idea of a good time—even when it’s digital, secure, and straightforward. But think of these standard information forms as the helpful blueprints that allow us to prepare complete, accurate income tax returns for you and your business. It’s the standard, routine process requested by tax and accounting professionals nationwide, and we’ve streamlined everything so you can breeze through it as painlessly as possible.
Here is a quick walkthrough of what we use, who completes it, and how often.
1. Tax Services Engagement Agreement
Status & Frequency: REQUIRED, once only (remains in force until or unless canceled by you or MKA).
Who completes it: All new clients.
What it’s about: This is our handy combination agreement that covers both individual/family returns and small business returns. It outlines the scope of work, keeps our relationship transparent, and sets clear expectations so we’re always on the same page.
2. IRS Consent to Disclose Source Documents Form
Status & Frequency: REQUESTED, annually. (Note: While mandated by annual IRS compliance rules, this is a quick, standard information form that simply requires your signature.)
Who completes it: All tax service clients.
What it’s about: This gives MKA your official consent to securely and confidentially share your source documents with our qualified team members or trusted contractors who assist in preparing your return.
3. Personal Annual Tax Organizer
Status & Frequency: REQUIRED, once only for new clients. For returning clients, we love making things effortless: in early January each year, MKA provides you with a pre-populated organizer. All you need to do is review it, update anything that has changed from last year, and sign off!
Who completes it: Individual and family tax clients—including small business owners who operate as single-member LLCs or sole proprietors. (Note: We keep this separate from business entity returns to keep things simple).
What it’s about: This organizer walks you step-by-step through gathering your personal income, deductions, and credits so no tax-saving stone is left unturned.
4. Small Business Annual Tax Organizer & Questionnaire
Status & Frequency: REQUIRED, once only for new clients. For returning business clients, MKA handles the heavy lifting by providing a pre-populated organizer every January so you can simply review, update any changes, and sign.
Who completes it: Clients operating a partnership, S-corporation, or C-corporation.
What it’s about: This focuses entirely on your corporate or partnership operations—tracking gross receipts, cost of goods sold, operating expenses, asset additions, and vehicle logs to ensure your entity return is dialed in.
A Quick Note on Our Secure Portal: Whenever we upload a new form, document, or your pre-populated annual organizer, an email notification is automatically triggered and sent straight to your inbox. You can easily review, sign, and upload documents right through your private, secure portal.
5. Client’s Authorization to Prepare Tax Returns
Status & Frequency: REQUIRED, annually for each tax year we prepare for you.
Who completes it: All tax clients (both new and returning).
What it’s about: Tax authorities require your official, signed authorization each year so MKA is legally permitted to prepare your return and handle designated administrative communications on your behalf.
