Request For Service Information

So that we can understand our client’s needs and provide the best client service experience possible, please select which services you are interested in from the following list. 

Please note that this is a form used for quality assurance only. By selecting a service category, you are not agreeing to engage our firm for those services and our firm is not agreeing to provide them. There is no cost or obligation of any kind when selecting a service category.

If you select “Yes” to any of the service categories below, our firm will provide you with information about that service including but not limited to the estimated cost to you for that service to the best of our understanding based upon the information that you may provide to us. Should you wish to engage our firm to provide that service, a separate engagement agreement will be provided to you for review and signature. 

If you select “No” to any of the service categories below, our firm will not provide you with information about that service at this time. You may contact us later should you change your mind and we will be very pleased to discuss that service, or any service that our firm provides with you at that time.
