Client’s Approval of Year End Financial Reports To Be Used To Prepare Tax Returns

MKA uploaded a DRAFT set of financial reports to your secure document sharing file including but not limited to profit and loss statements, balance sheet, etc. for your review. All transactions that are included on those reports were prepared using information that you provided to our firm from bank statements, credit card statements, etc. We have neither added to nor subtracted information from those sources. We have recorded the transactions according to your instructions.

Now that you have received those draft reports, it is important that you review them carefully and provide us with guidance relative to any changes that you feel should be made. When you receive the updated, FINAL financial reports, or if no changes are required to the drafts that we have uploaded, we will ask that you sign the attached approval form to indicate your acceptance of those report and our bookkeeping for your business.

Please keep in mind that the final and approved set of financial reports for your business will be used to prepare tax returns, either by our firm or another tax service, and can impact the tax return and your tax liability. Please look them over carefully and let us know if you have any questions at all.

Additional charges and fees may apply for any accounting and bookkeeping changes and adjustments requested or required after January 31st.

By signing below on behalf of my business organization, I acknowledge and confirm the following:

  • I have received a final set of financial reports for my business and have reviewed it carefully.
  • I have had an opportunity to review the bookkeeping work completed by MKA and confirm that it accurately and completely represents all revenue and expense transactions that pertain to my business.
  • I have had an opportunity to discuss the final financial reports prepared by MKA with the bookkeeper and confirm that any questions or concerns have been answered to my satisfaction.
  • I understand that the final and approved set of financial reports prepared by MKA will be used as a basis for preparing tax returns and may have an impact on my tax liability.
  • I confirm that I have truthfully, to the best of my ability, reported all revenue, expense, asset and liability information completely and accurately to MKA, and that such information has been accurately captured and recorded on the approved set of financial reports for my business.
  • I confirm that I have not asked for and have not received legal advice from MKA and that all communication pertaining to accounting or bookkeeping services was limited to accurate recording of financial transactions in QuickBooks.
  • I indemnify and hold MKA harmless for any costs or claims or damages that result from misleading, inaccurate or incomplete information that I may have reported to MKA. I take full responsibility for all information reported to MKA and for the financial reports that were generated by or from such information.
  • I am authorized to accept and approve financial reports on behalf of my business.
