- New Client Information Questionnaire – Required for all payroll clients
- New Employee Information Questionnaire – Required for all new employees added to the payroll service
- IRS Form 8655 Reporting Agent Authorization – Required for all payroll clients
- New Employee Direct Deposit Authorization Form – Required for all new employees added to the payroll service
- IRS Form W4 Employee Withholding Allowance Certificate – Required for all new employees added to the payroll service
- IRS Form I9 Employee Eligibility Verification – Required for all new employees added to the payroll service
- IRS Form W9 Request for Taxpayer ID & Certification – Required for all independent contractors added to the payroll service
- Engagement Agreement for Annual 1099 Services – Required for all business tax clients (if applicable)
- Credit Card Authorization Form – Required for all payroll clients
- All Services Opt-in/Opt-Out Form – Required for all clients
